You're standing at Santa Maria presso San Satiro, a church in Milan. The name means Saint Mary near Saint Satyrus. Sounds odd, but it's spot on: this Renaissance church was built on the site of a much older shrine, which is still part of it. Take a look at the bell tower. It's Romanesque, so older than everything around it. It belonged to the church that stood here before the whole thing was rebuilt in the late fifteenth century. Above the nave you'll see a round dome, right where the transept crosses it. The baptistry attached to the church also dates from the fifteenth century.
